How to Enroll in
COMMUNITY Charter School

Thank you for considering COMMUNITY Charter School for your child!

Admission to COMMUNITY Charter School is free and open to all students eligible to attend public school in Erie County. However, because we're located within the Buffalo School District, Buffalo residents are given preference for enrollment.

If you're interested in enrolling your child in COMMUNITY Charter School, please contact us online or call us at 716-833-5967.

We will then mail you a registration packet, which includes the necessary enrollment and transportation forms, as well as information about our school's policies, including the Dress Code Policy and Code of Conduct. Once you complete the registration packet, you can mail it back to us or drop it off at COMMUNITY Charter School's Main Office at:

The following documents must also submitted with your registration packet.

  1. Child's shot record (Copies accepted)
  2. Birth Certificate (Copies accepted)
  3. Physical Exam
    Kindergarten physical must be received in the Main Office prior to the first day of school. Your child's BMI (Body Mass Index) must be included on the physical form. Students will not be allowed to enter school without updated physical or vaccination forms.
  4. Dental Exam
    Only required in Grades K, 2, and 4.
  5. Final Report Card or Transcripts
    Only required in Grades 1-5.
  6. Proof of Address
    • Current gas or electric bill
    • Driver's license / Non-driver's ID
    • Pay stub or employee statement
  7. BMH lease
  8. Medical Requirements
    All students entering Kindergarten, 2nd grade, 4th grade, and all new students are required by New York State to provide a documented physical, immunization record, and dental certificate prior to the start of school.

    All 6th graders are required by New York State to have documentation of Tdap immunization prior to the start of school unless they are under 11 years of age. Tdap is a vaccine for adolescents that protects against tetanus, diphtheria, and pertussis. Students are expected to have the Tdap immunization within 14 days of their 11th birthday and provide documentation to remain in school.


We will not enroll your child into COMMUNITY Charter School until you have provided us with all of the necessary documents listed above.

Note: You can request items 1-5 from your child's current school. All medical records can be requested from your medical providers.
